FOR YOUR APPOINTMENT
If you are a new patient, please arrive to your appointment early in order to complete the registration process. In addition, please bring the following:
- Driver's License or State ID
- Referral (If Required by Insurer)
- Insurance Information
- Copies of Operation Records, Medical Records, X-rays, MRIs, and CT Scans from Prior Doctor Visits
- A List of Current Medications and Any Known Allergies
Insurance Information
Patients are responsible to make sure the physicians of Sports Medicine Oregon are participating providers for their insurance plans. Please be aware that some or all of the services provided may be considered by your insurance to be “non-covered” services and may not be considered medically necessary under your plan’s provisions. You will be responsible for these charges. Please check with your carrier or your plan handbook for information regarding your covered and non-covered services.
To verify if our physician is in network with your insurance plan, please call the phone number on the back of your insurance card. The insurance representative will be able to confirm if our provider is a contracted provider as well as explain your insurance benefits.
We accept most major insurance plans including the following:
- Blue Cross (Regence)
- Cigna/Great West
- First Choice Health Network
- Healthnet
- Medicare
- Moda
- Pacificsource
- Providence Health Plans (Some plans require referrals.)
- Providence Preferred Network
- Tricare Standard and Prime/Remote (Some plans require referrals.)
- UMR
- United Health Care (Navigate Plan requires a referral)
Appointments
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
Fees & Payments
We make every effort to decrease the cost of your medical care. We accept cash, checks, MasterCard, Visa, Discover and CareCredit for your convenience. Copayments are due at the time of service. We will help assist you with all billing matters. However, remaining balances are the patient’s responsibility.
Prescriptions & Renewals
Please request all prescriptions and authorizations during our regular office hours, when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances. Please allow 48 hours for all prescription refill requests.
